If the student did not play a fall and/or winter sport at PV please click here to access and print ALL required PIAA and PVSD forms. Every form must be completed, signed and given to the coach prior to the start of practice in order to participate. No exceptions.
If the student did play a fall and/or winter sport at PV please click here to access and print required parent recertification (section 7) and other forms. If the student did not finish the fall season due to injury/illness, do not use this form. Please access the Section 8 forms below
If the student played a fall and/or winter sport at PV and was injured or experienced illness that prevented them from finishing the season, please click here for physician recertification (section 8) and other forms.
Please note: All physical exams must be dated after June 1st and before the start of practices each school year before an athlete can participate. Parents/Guardians are encouraged to schedule annual check-ups/physicals within the June 1 - August 1 window in order to avoid potential insurance limits and co-pays.
Do NOT fax forms! We must have the original forms with signatures for our files. Copies cannot be accepted.
